Medicare and Medicaid
If you have Medicare or Medicaid, please contact us at Seattle, WA 888-680-4140 or Milwaukee, WI 888-611-3904, and we will file a claim to
the appropriate Medicare or Medicaid agency.
Filing Primary Insurance
If you have other insurance coverage, please call us at Seattle, WA 888-680-4140 or Milwaukee, WI 888-611-3904. If Dynacare has a direct billing arrangement with your insurance company, we will file the claim to your insurance company. If Dynacare does not have a direct billing arrangement with your insurance company, we will ask you to remit prompt payment directly to Dynacare. We will be glad to send you a health insurance claim form so you may file a claim for reimbursement directly with the insurance company. To ensure that your claim is filed correctly, please have your Medicare, Medicaid, or other insurance card available when you call Dynacare.
Filing Secondary Insurance
You may be covered by more than one insurance plan. Please contact your insurance
company if you are uncertain whether it has a direct billing arrangement with Dynacare.
If you have Medicare as a secondary payor, you may be asked to submit the explanation
of benefits from your primary plan directly to Dynacare in order for Dynacare to file the
secondary claim to Medicare. If Dynacare did not submit the claim to your primary payor,
Dynacare will need the primary payor’s explanation of benefits to file your claim for
services to Medicare. If Dynacare submitted the primary claim to your insurance company,
Dynacare will file the secondary claim to Medicare on your behalf.
If you have Medicaid as your secondary coverage, please contact Dynacare directly for
filing the claim to Medicaid. If Dynacare does not have a direct billing arrangement with your secondary payor, you will need to mail a copy of your invoice and a copy of the explanation of benefits from the primary payor to the secondary payor at the address listed for your secondary plan.
